The National Library of Medicine (U.S.) recently published the 1999 
edition of its "Directory of History of Medicine Collections", which
provides contact information and collection scope and highlight notes 
for several dozen libraries, archives, and museums in the United 
States, Canada, and the UK.  Single copies are available free of charge
