Hi,
To all of you who have patient records in your archives, I would
appreciate some advice, guidance, words of wisdom, etc., on how you
handle requests for information from these records. Do you have a
policy or a procedure (Institutional Review Board?) or forms for such
requests? If so, would you mind briefly describing them?
